Attestation Handling Procedures

MAP Mail-in Attestation Applications can be submitted one of two ways:

  1. By mail to Central Health Eligibility Services at PO BOX 300489 Austin, Texas 78703, or by fax to 512-901-9701.
  2. Utilizing the VeritySource database’s “Contacts” module-on hold 6-2-21

Preparation of the MAP Attestation applications received through the mail:

  1. Date stamp each page of the application, the envelope, and any other documents that may be included with the applications using the date the application was received
  2. Staple multiple page documents together
  3. Determine the status of the application and sort applications as:
    1. Complete application
      1. All questions are answered
      2. The Attestation is signed and dated
      3. A new Applicant Responsibilities for all adults in the household is signed and dated
      4. Received within 30 days of the application received date.
    2. Incomplete application
      1. All or some questions are unanswered
      2. The Attestation is not signed and dated
      3. A new Applicant Responsibilities for all adults in the household  is not signed and dated
      4. Not received within 30 days of the application received date.

Processing MAP Attestation Applications:

  1. Verify that the application is complete as per step 3 above.
  2. Verify the applicant’s coverage:
    • If the client does not have active MAP coverage, but had active coverage within the last 30 days proceed to step 3.
    • If the client does not have active MAP coverage, but had active coverage more than 30 days ago applicant must reapply.
  3. Utilize the instructions found in VeritySourceTM User Manual Chapter 5G Processing Mail-In Attestations to process the application.
  4. If application is complete, coverage will be extended up to an additional 6 months from the original termination date not to exceed one year total coverage.
  5. If application is incomplete send a denial letter with instructions to apply online or schedule an in-person appointment.
  6. When a PCP change request has occurred and an attestation is later submitted look at the original effective date to determine when the expiration date should be.
  7. When a PCP change is requested after an attestation has been approved remember to mark the track with the custom attribute of attestation and the re-approval date so that another attestation is not sent out to client.
  8. Print card and mail to applicant with program information.
